Belt buckle: There is some debate over what the actual buckle looks like, but it is a specific stylized skill with horns coming out of the sides, possibly with spiked bands around the base of the horns, and is very shiny chrome/silver (not dirty or dull). Terrance has said it was his tribute to Danzig and would have used a Danzig buckle if he could, but wasn't able to due to copyright. It is the buckle most widely used and accepted by fans.
Belts/chains: "black leather x 2 - one with skull buckle, one with plain double pronged buckle and many wallet chains hooked onto the belt holes"
It also looks like he may have some skull accents on his second belt (the one that has a double-pronged buckle and has the Zydrate Gun Holster attached to it) if you look closely during "Come Up And Try My New Parts"
Other skull accents can be seen on the belt in these photos:
Knee pad: "black knee-pad worn on one leg"
Does not appear to have a hard plastic piece on it like many knee pads are made with, and has some paint and other general weathering/dirtiness applied to it. Also wraps around the whole leg.
Scarf: "cheese cloth dyed navy blue"
Very simple. Will roll up after being dyed and washed, this is a good thing and will create the right look and texture.
Zydrate Bandoleer: Not talked about a lot, but likely made from a modified "US Military Police Sam Brown sling." Compare the photo of the prop to the photo after it, and a link is provided to the store selling it. A real pain to modify for the cost of the sling, likely cheaper and easier to make one from scratch.

Black elastic can be used in place of leather if you are worried about the vials slipping out. Just stretch it over the vial when sewing the elastic on, so it will stretch tight and hold the vials in place.
Monocle: Another item rarely talked about, and little is known. Gold or copper in color, monocles are cheap and easy to find at Party City or ebay. The one in the movie has a handle, more like a tiny magnifying glass.
